ΤΠΥ με παρακράτηση φόρου και εξαίρεση ΦΠΑ

Καλησπέρα σας,
πελάτης μου Ιατρός πρέπει να κόψει ΤΠΥ χωρίς ΦΠΑ και με παρακράτηση φόρου 20%.
Έχει κάποιος κάποιο παράδειγμα? Αυτό που στέλνω μου επιστέφει το παρακάτω λάθος:
